The VA Midwest Health Care Network (VISN 23) serves more than 430,000 enrolled Veterans with the majority of Veterans residing in the states of Iowa, Minnesota, Nebraska, North Dakota and South Dakota.
A Veterans Integrated Service Network (VISN) Telehealth Hub Primary Care Nurse Practitioner (NP) is responsible for the overall provision of medical care with a Patient Aligned Care Team (PACT) serving eight Health Care Systems in VISN 23. This position will primarily function to provide Primary Care gap coverage to various Health Care System's throughout the VISN using Telehealth modalities but may include face-to-face care intermittently. Occasional travel may be required to clinics within the Network for in-person clinics. The Nurse Practitioner in this position will effectively function as a team member of a PACT to provide high-quality care to Veterans using Telehealth modalities. In addition they will maintain and respond to any Clinical Reminders that are identified during a visit and be responsible for the performance of clinical duties within the scope of a primary care community-based outpatient clinic on a regularly scheduled basis. The Telehealth Hub NP will demonstrate knowledge, skills and abilities to conduct primary care exams through Telehealth modalities as well as coordinate patient care longitudinally. An important part of the position is to maintain effectiveness in recognizing emergency/acute situations which could occur remotely and intervene in an appropriate and effective manner. It is expected that the NP will document patient health care clearly and within prescribed time frames and complete all forms associated with the clinic visit.

Education/Experience Requirements: A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a masters degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body.The certification must be in the specialty to which the individual is being appointed or selected.
