This full-time, permanent position is located at the Conroe Outpatient Clinic, under the Rehabilitation and Extended Care Line (RECL). The incumbent serves as a liaison in the Home Based Primary Care (HBPC) and functions as member of a multidisciplinary specialty care team and other patient care teams as needed to provide quality, effective, and efficient patient centered care.
Duties include, but are not limited to:
-Reviewing medical records and independently evaluating patients (including utilization and interpretation of functional tests and measurements) to determine functional deficits and the best method of accomplishing rehabilitation goals.
-Accurately completes functional assessments using approved measures (FIM).
-Completes patient and/or family training as indicated.
-Initiates prosthetic request for equipment and devices.
-Documents results of evaluations, patient treatment including patient/family education, and patient's response to treatment in accordance with MEDVAMC, RECL, policies and TJC standards.
-Attends HBPC interdisciplinary team meetings, family conferences, and staff meetings when needed.
Work Schedule: Monday-Friday, 8:00am to 4:30pm, Some weekend work may be required at the MEDVAMC in Houston.
