Nurse Practitioner provides professional assessment, coordination and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received and that progress is being made toward care goals. The scope of the coordination of care will be throughout the continuum of care to assure that care is timely, appropriate, of high quality and cost effective. In addition the Nurse Practitioner provides ongoing monitoring and evaluation of care management services.
The Nurse Practitioner provides direct and indirect wound care nursing care to patients and families in the outpatient area. S/he is accountable for adhering to established policies, standards of care and standards of practice. The incumbent demonstrates leadership when providing and improving patient care in an outpatient setting. The Nurse Practitioner reports directly to the RTCL Medical Director of the patient care unit/area. The Nurse Practitioner serves as a role model when functioning as a clinical consultant, content expert. The Nurse Practitioner runs wound care clinics and provides specialized evidence based treatments for non-healing ulcers and wounds to the outpatient wounds. The population includes: venous/arterial ulcers, diabetic ulcers, and traumatic and non-healing surgical wounds. This diverse clinic picture necessitates that the incumbent possess current knowledge of medical/surgical issues as well as advanced wound care treatment modalities, including, but not limited to: biological tissue grafts; low frequency ultrasound treatments; compression wraps; lymphedema pumps; topical oxygen; and specific wound dressings. The wound care Nurse Practitioner also collaborates with physicians, therapists, registered nurses, social workers, and home care agencies and provides the most current scientific-bases wound treatments modalities from in-patient to long term care and/or home care settings. The incumbent is responsible for the education of caregivers.
While working with Home Telehealth (HT) the Nurse Practitioner will work collaboratively with primary care providers and other healthcare professionals, team members, clinics, internal or external services, and community agencies. In addition the Nurse Practitioner provides ongoing monitoring and evaluation of care management services. The Nurse Practitioner will provide consultation to Home Telehealth care coordinators on medical management of patients, write orders as needed following Home Telehealth standard operating procedures and the National Home Telehealth Operations Manual and Conditions of Participation. The Nurse Practitioner will require knowledge of chronic disease management and end-stage disease conditions, for example: heart failure, chronic obstructive pulmonary disease, hypertension, and diabetic management. The incumbent will work collaboratively with the Home Telehealth Lead Care Coordinator to screen consults, will participate in Home Telehealth performance improvement projects and in-services, and provide education and outreach to other areas and disciplines across the hospital.
Work Schedule: Monday - Friday, 8:00 a.m. to 4:30 p.m.

Physical Requirements: This position requires heavy lifting, 45 pounds heavy carrying, 45 pounds straight pulling (2 hours); reaching above shoulder; use of fingers; both hands required; walking & standing 8 hours - intermittently; repeated bending; both legs required; ability for rapid mental& muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20 and to 20/40 in the other; depth perception; ability to distinguish shades of colors; hearing (aide permitted). Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.

Must provide a written statement into the privileging (scopes in Tucson) which attests to the candidate's experience and time in practice, the types of procedures, and what the NP will do to maintain skills in this area of practice.
Minimum Requirement: 1 year chronic disease management experience.
Preferred Experience: Wound care certification, 2 years of experience with direct wound care and 1 year experience with Telehealth.
