This position is located at the Stratton VAMC in Albany, with occasional travel to the outlying Community Based Outreach Clinics. The NP must possess an active license in any state or territory of the United States and must maintain privileges at all times at the Albany Stratton VA Medical Center to be able to work as an NP. All primary care NPs must maintain current American Heart Association BLS Certification.
The NP for the Patient Aligned Care Team:
Executes position responsibilities that demonstrate leadership, experience, and creative approaches to the management of complex client care.
Functions autonomously within a defined scope of practice and ongoing collaboration with physician and interdisciplinary team.
Is a member of the team assigned to provide co-located collaborative care to promote effective treatment of common mental health conditions in the primary care environment (e.g. mild to moderate depression, problem drining, anxiety, and adjustment disorders).
Responsibilities include screening and assessment, diagnosis, pharmacological interventions, individual counseling, and consultation with PACT team, patient, and families to address treatment needs.

BASIC REQUIREMENTS:
· U.S. citizenship.
· Proficient in spoken and written English as required by 38 USC 7402(d) and 7407(d)
· Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
· Current, full, active, and unrestricted registration as a Nurse Practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
PREFERRED EXPERIENCE:
· American Heart Association Basic Life Support (BLS) for the Healthcare Provider Certification.
GRADE DETERMINATIONS:
Experience as a RN will be evaluated to determine if the experience is of an acceptable level of quality with regard to the following four dimensions of nursing: Practice, Professional Development, Collaboration and Scientific Inquiry. Each dimension of practice has criteria that demonstrate essential core competencies for VHA Nursing Personnel. The criteria are as follows: Practice: practice, ethics and resource utilization; Professional Development: performance, education/career development; Collaboration; collaboration, collegiality; Scientific Inquiry: quality of care, research.
