HUD VASH social work case manager is assigned to Social Work Services and work is performed independently within the Homeless Veterans Program, located on the campus of the VA Long Beach Healthcare System, Long Beach, California. HUD VASH program is located at multiple sites- the main medical center, Long Beach and Santa Ana clinic The HUD VASH social work case manager may be assigned to either location based on needs of the program. However, a primary base location is identified based on the number of HUD vouchers assigned to both locations. The HUD VASH social work case manager provides services specifically directed toward homeless Veterans and their families or Veterans at-risk for homelessness who meet eligibility requirements for a HUD voucher. Work is conducted in non-traditional office settings, completes independent field visits as a part of case management in Long Beach and/or Orange County, the functions of the position require local travel and the HUD VASH social work case manager must be able to independently operate a government vehicle and possess an active/valid state driver's license to perform these functions.
Application demonstrates post-graduate work history as a social worker in a clinical healthcare setting, working independently with a specialized population- adults who are homelessness/at-risk for homelessness, chronic multiple mental health/physical health conditions and/or substance use; must have a state's highest independent social work licensure; must have or be able to attain/perform Basic Life Safety-Healthcare skills.
Duties:
· Serves as the primary contact with Long Beach and/or Orange County Public Housing Authority (PHAs).
· Interviews, screen and assess Veterans to determine if HUD voucher eligibility requirements can be met.
· Develops a short-term treatment with the Veterans to gather required documents and submit to the PHA within target dates.
· Completes initial and annual psychosocial assessments; In coordination with the Veteran, develops short-term and long term housing treatment plan.
· Develops housing focused treatment plan, once a voucher is received, with the Veteran to locate and have a set move in date within 120-days after receipt of HUD voucher.
· Develops a long term case management treatment plan, with Veteran input, once housing is attained; evaluating progress towards goals and adjust the treatment plan as appropriate.
· Documents independently using an electronic medical record for scheduling appointments and documenting services provided.
· Daily input using paper and/or electronic formats for reporting and tracking software throughout the case management continuum
· Demonstrates standardized case management practice skills.
· Provides referrals to VA medical and mental health inpatient and outpatient treatment and to other VA services as well as community-based social services and other non-VA entitlement programs.
Uses the Housing First Model approach and shared team concept based on 4-levels of case management
All social workers in the 0185 series participate in discipline specific Integrated Peer Review; social workers meeting qualification requirements may participate in the graduate social work student program; participate in discipline specific training, activities; Per VA Directive 2010-10 Standards for Emergency Department and Urgent Care Clinic Staffing Needs in VHA Facilities, social worker may need participate in a rotated scheduling to provide telephone consult to the Emergency Department after normal work tours.

Per VHA Directive, all qualified social workers are responsible to provide after-hours on-call to the Emergency Department. Qualified social workers may volunteer for a calendar year on-call rotation schedule. In the event there are not enough volunteers, all qualified social workers will be assigned and rotated to the calendar year on-call schedule by the Chief, Social Work Services. Compensation is provided when after hour's on-call.
This medical center operates 24 hours, seven days per week and you may be required to work days, evenings, nights, holidays, and/or weekends, to include modification in shifts and/or tours of duty, to meet medical center and patient care needs.
Physical Requirements:Applicants must be physically and mentally able to efficiently perform the essential functions of the position without hazard to themselves or others; Light lifting (under 15 pounds) light carrying (less than15 pounds); walking (.5 hours); standing (.5 hours); kneeling (.5 hours); repeated bending (.5 hours); entering data and documentation into computer systems (2.5 hours); may require field visits for continuity of care- ability to drive a vehicle (must have valid driver's license) Environmental Factors :Outside and inside, Working closely with others, including patients with physical or emotional conditions that maybe unpredictable, Working independently.
Basic Requirements:
Citizenship. Be a citizen of the United States.
Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to the CSWE website to verify if that social work degree meets the accreditation standards for a master of social work.
Licensure. Persons hired or reassigned to social worker positions in the GS-185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found on the OHRM website. (Copy of license required)
GRADE DETERMINATIONS:
Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. NOTE: Licensure or certification at this level is required and a copy MUST BE PROVIDED.
OR,
In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs in subparagraph (b) below.
(b) Demonstrated Knowledge, Skills, and Abilities
1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.
2. Ability to independently assess the psychosocial functioning and needs of patients and their family members and to formulate and implement a treatment plan, identifying the patient's problems, strengths, weaknesses, coping skills and assistance needed, in collaboration with the patient, family and interdisciplinary treatment team.
3. Ability to independently conduct psychosocial assessments and provide psychosocial treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. This requires knowledge of human development and behavior (physical and psychological) and the differential influences of the environment, society and culture.
4. Knowledge and experience in the use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
5. Knowledge of psychosocial treatment and ability to independently implement treatment modalities in working with individuals, families and groups who are experiencing a variety of psychiatric, medical and social problems to achieve treatment goals. This requires independent judgment and skill in utilizing supportive, problem solving or crisis intervention techniques.
6. Ability to independently provide counseling and/or psychotherapy services to individuals, groups and families. Social workers must practice within the bounds of their license or certification. For example, some states may require social workers providing psychotherapy to have a clinical level of licensure.
7. Ability to provide consultation services to other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Ability to provide orientation and coaching to new social workers and social work graduate students. Ability to serve as a field instructor for social work graduate students who are completing VHA field placements.
8. Ability to independently evaluate his/her own practice through participation in professional peer review case conferences, research studies, or other organized means.
9. Knowledge and skill in the use of computer software applications for drafting documents, data management, and tracking, especially those programs in use by VHA.
