AVAHCS
currently offers general medical and surgery inpatient care, day surgery, long
term care, home based primary care and specialty care, mental health services
including substance abuse treatment, homeless services and an OEF/OIF program
to a population of 64,400 veterans in a primary service area that covers
counties in the Texas Panhandle, northern Oklahoma Panhandle and eastern New
Mexico. We are a level 2 facility serving 25,602 unique Veterans. In FY12,
there were 2,315 inpatients treated and 264,380 outpatient visits. There are
currently 45 total operating hospital beds, which includes 14 ICU beds and 120
operating community living center beds. The FY13 annual budget is $180+million.
Our facility provides an active teaching environment for medical students and
residents through a formal affiliation with Texas Tech University Health
Sciences Center. The rotation includes approximately 50 residents in Internal
Medicine, Family Practice, and Specialty Care. Other affiliations provide a
variety of educational opportunities in pharmacy, optometry, nursing and
various allied health programs.

The Amarillo VA Health Care System is seeking a Nurse Practitioner for Cancer Care Navigator Team (CCNT). Experience in hemotology/oncology is preferred.
Duties include:
- Knowledge and active participation in unit/program level
quality improvement processes, initiatives and customer service programs
- Assumes responsibility for the coordination of care
focused on patient transition through the continuum of care: Suspicion to
Diagnosis and Treatment
- Serves as reference to PACT teams/specialty teams to
facilitate timely, appropriate work-ups that optimize VA resources, facilitating
community care as needed
- Inform patient/family of process, general disease
education
- Track suspicious imaging and pathology reports to
conclusion, assuring timely review by specialists
- Ensure timely Tumor Board review in collaboration with
other VISN CCNTs, gather care plan from Tumor Board review and support
communication of plans to PACT team and patient/family
- Identify high risk patients by acuity and/or
biopsychosocial issues for intensive case management
- Monitor activities to ensure timely multidisciplinary
review and coordination of activities necessary for development of a treatment
plan and necessary supportive procedures (e.g. placement of access/IV lines,
dental extractions, etc.).
- Assess and link patient to additional services when
needed, i.e. alcohol treatment, nutrition, palliative care, hospice as
necessary
- Communicate/coordinate with PACT team re: medications,
patient care needs, etc.
- Support patient education, treatment education, awareness
of steps in care.
- Assess and coordinate treatment for symptom management,
and/or refer to appropriate specialty for higher level care needs
Work Schedule: 8:00-4:30
Physical Requirements: The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing, reaching (overhead, extensive/repetitive), repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking.
BASIC REQUIREMENTS:
-U.S. citizenship. Non-citizens may be appointed when it is not possible
to recruit qualified citizens in accordance with VA Policy.
-Full unrestricted Licensure, Certification, or Registration required.
-Graduate of a school of professional nursing approved by the appropriate
State-accrediting agency and accredited by one of the following accrediting
bodies at the time the program was completed by the applicant: The
Accreditation Commission for Education in Nursing (ACEN) or The Commission on
Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools
of professional nursing, possession of current, full, active and unrestricted
registration will meet the requirement of graduation from an approved school of
professional nursing. The completion of coursework equivalent to a nursing
degree in a MSN Bridge Program that qualifies for professional nursing
registration constitutes the completion of an approved course of study of
professional nursing. Students should submit the certificate of professional
nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript.
(Reference VA Handbook 5005, Appendix G6).
-Current, full, active, and unrestricted registration as a graduate professional
nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United
States, or the District of Columbia.
-Nurse Practitioners. A nurse practitioner must be licensed or otherwise
recognized as a nurse practitioner in a State, possess a master's degree from a
program accredited by the NLNAC or CCNE and maintain full and current
certification as a nurse practitioner from the American Nurses Association or
another nationally recognized certifying body. [The certification must be in
the specialty to which the individual is being appointed or selected.
-English Language Proficiency. RNs appointed to direct
patient care positions must be proficient in spoken and written English.
Preferred Experience: Oncology/Hemotology
Experience refers to paid and unpaid experience, including
volunteer work done through National Service programs (e.g., Peace Corps,
AmeriCorps) and other organizations (e.g., professional; philanthropic;
religions; spiritual; community; student; social). Volunteer work helps build critical
competencies, knowledge, and skills and can provide valuable training and
experience that translates directly to paid employment. You will receive credit for all qualifying
experience, including volunteer experience.
Grade Determinations: Experience as a RN will be evaluated to
determine if the experience is of an acceptable level of quality with
regard to the following four dimensions of nursing: Practice,
Professional Development, Collaboration and Scientific Inquiry. Each
dimension of practice has criteria that demonstrate essential core
competencies for VHA Nursing Personnel. The criteria are as follows:
