The El Paso VAHCS is part of VISN 17 Network, and is a complexity Level 3 standalone ambulatory care facility with over 800 employees and growing. El Paso VA Health Care System serves Veterans in far Southwest Texas and Dona Ana County, New Mexico. The incumbent will serve as a Nurse Practitioner in Primary Care for the Patient Aligned Care Team (PACT).
· Works with a multidisciplinary team including physicians, nursing, social work, dietician, clinical pharmacist, behavioral health and clerical support.
· He/she will perform and record history and physical examinations and record data from clinic visit.
· Evaluates, consults, develops diagnosis, and manages the primary care panel of patients assigned.
· Enters progress notes in computerized medical record and completes workload documentation in a timely manner.
· Provides medical care encompassing acute and chronic medical conditions and all appropriate preventive care for patients.
· Order diagnostic tests, medications, laboratory tests as medically relevant for each patient.
· Provides timely follow-up on results, including completion of view alerts.
· Determines medical assessment and develop treatment plans according to accepted protocols.
· Prescribes medication according to approved formulary, and in accordance with Federal and State Law.
· May collaborate with an assigned mid-level provider and complete collaborative case reviews as assigned.
