The Nurse Practitioner Hospice and Palliative Care Coordinator (HPCC) position is located in the Geriatrics and Extended within VA Tennessee Valley Healthcare System. The Nurse Practitioner is under the supervision of the attending Physician who in turn is under the direct supervision of the Chief of Geriatrics and Extended Care. The HPCC is primarily responsible for implementation of the Palliative Care Program, ensuring high quality practices in the treatment of symptoms at the end of life through the coordination and integration of various nursing, medical, and complementary interventions. The HPCC will collaborate with the VISN Hospice and Palliative Care Coordinator. Responsibilities will include coordination of the efforts of the multiple disciplines involved in caring for patients at the end of life with the Palliative Care Team (PCCT) and the inpatient hospice and palliative care patients serving as a consultant and resource person for the PCCT, the hospital staff, TVHS leadership, and VISN leadership; development and implementation of educational programs; and development and implementation of specific measures to monitor the effectiveness of the program. The Nurse Practitioner demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. The NP is integrally involved in the assessment, planning, intervention, evaluation and revision of a plan of care for patients, integrating appropriate technologies and communicating with appropriate personnel. The NP acts o behalf of the veteran to assure that necessary clinical services are received and are veteran centered. Position may entail other duties as assigned.
Travel between the Nashville Campus and the Alvin C. York (Murfreesboro) Campus is required.
Certification as a Nurse Practitioner is required
Nurse Practitioner License and RN license is required
Preferred Qualifications:
Five years clinical experience in medical surgical nursing with at least four years of clinical experience in the Nurse Practitioner role Preferred.
Experience in hospice/palliative care is preferred
Experience in program development and oversight preferred
Basic Life Support (BLS) certified preferred
Physical Standards/Requirements: All applicants must be physically and mentally able to perform efficiently the duties of the position. Must be able to lift (30 lbs or more); twisting and/or bending, standing and/or walking almost constantly during the course of the day.
BASIC REQUIREMENTS:
In addition, a Nurse Practitioner must be a graduate of a school of professional nursing in a Nurse Practitioner Program with Masters Degree in Nursing (NLNAC) (ACEN) or CCNE accredited) with current certification as a nurse practitioner [The certification must be in the specialty to which the individual is being appointed or selected] from the American Nurses Association or another nationally recognized certifying body (such as the American Academy of Nurse Practitioner AANP). In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing
