     VA TEXAS VALLEY COASTAL BEND HEALTH CARE SYSTEM

    The VATVCBHCS is comprised of the Corpus Christi Outpatient Clinic (CCOPC), Corpus Christi Specialty Clinic (CCSC), Harlingen Outpatient Clinic (HOPC), Health Care Center at Harlingen (HCC), Laredo Outpatient Clinic (LOPC), and McAllen Outpatient Clinic (MCOPC). The VATVCBHCS is an ambulatory health care system providing health care in medicine, surgery, psychiatry, and rehabilitation medicine. The VATVCBHCS also operates a 100-patient Home Based Primary Care program and a 200-patient Care Coordination Home Tele-Health Program. Through the utilization of inpatient/emergency department contracts, approximately 2600 inpatient admission and 5800 ER visits are managed by VATVCBHCS. Affiliated with the University of Texas Regional Academic Health Campus, the system has an active ambulatory care program and serves as a teaching and training site for medical residents. The VATVCBHCS is responsible for an enrollee population of over 30,000 veterans and 350,000 outpatient visits annually.

    Responsibilities

    The selectee for this position will serve as a Housing and Urban Development - VA Supportive Housing (HUD-VASH) Registered Nurse Case Manager for the VA Texas Valley Coastal Bend Health Care System's (VATXVCBHCS) Corpus Christi Outpatient Clinic (CCOPC). Selectee will provide case management for a caseload of Veterans in the HUD-VASH program. Case management is defined as the provision of services by VA clinical staff to homeless Veterans and includes all activities to provide appropriate treatment and maintain Veterans in HUD approved stable housing. HUD-VASH is an intensive case management program for chronically homeless Veterans aimed at providing psychosocial services to Veterans while residing in stable housing through HUD's Section 8 voucher system. The Nurse (selectee) in this program will function as a member of a multi-disciplinary team providing services to Veterans with multiple medical and mental health issues. The nurse provides comprehensive discipline-specific services to Veterans and their families. Responsibilities include the management, coordination and provision of discipline-specific services to Veteran and their families in inpatient, outpatient and community settings. Selectee will utilize advanced skills to assess/evaluate, identify problems while providing psychiatric mental health care that promotes wellness and prevents illness and disability. The selectee will also assess Veterans' current medical problems and intervene, accordingly.

    Duties include the following, but are not limited to:

      

    • Reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements.

    • Performing site visits of HUD-VASH apartments to ensure that Veterans reside in safe environments in compliance with local housing codes.

    • Verifying the Veteran's status, eligibility for VA medical care, and eligibility of program participation.

    • Ensuring homeless Veterans are considered as primary participants for program acceptance.

    • Collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures.

    • Coordinating care including serving as an advocate on behalf of the Veterans to ensure comprehensive service delivery, and linking and referring Veterans to the Health Care System, VA Regional Offices and/or community agencies.

    • Complying with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager.

    • Actively participating in HUD-VASH Program conference calls and attending training offered by the HUD-VASH.

    • Following local policy and SOP for the HUD-VASH Program. This includes following policies and documentation requirements as required for CARF and Joint Commission accreditation.

    • Participating in program planning and development, advising the supervisor of shifting trends with caseloads and identifying the need for and making recommendations on program procedure or policy changes based upon work experiences, etc.

    Work Schedule: Monday - Friday, 8:00 a.m. – 4:30 p.m.; Alternate schedules may be necessary to meet the needs of the organization.

    Physical Requirements: The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing, reaching (overhead, extensive/repetitive), repetitive motion and stooping.  The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking.  The environment's pace can vary from steady to a sometimes fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. 

    Qualifications

    BASIC REQUIREMENTS:  
    • U.S. Citizenship.  Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
    • Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:  The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).  In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing.
      • The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing.  Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6)
    • Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. 
    Grade Determinations:  The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
    • Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
    • Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
    • Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
    • Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. 
    • Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree in nursing or related field and approximately 2-3 year's of nursing practice/experience.

    Preferred Experience:

    ·       2 - 3 years case management experience preferred

    ·       1 - 2 years mental health experience preferred
